Provide case management services for older adults who are at risk for institutionalization. The Multipurpose Senior Services Program (MSSP) is a Medi-Cal funded program offered locally by the Fresno-Madera Area Agency on Aging with no charges for services. MSSP eligibility requires that the individual be age 65 and older, receiving Medi-Cal benefits, frail & at risk for nursing home placement, reside in Fresno or Madera area and have a need for care management and be willing to participate in the program. MSSP is a multidisciplinary care management program coordinated by a public health nurse and social worker. It provides a comprehensive health and psychosocial assessment that takes place in the client's home. An individual plan of care is developed in collaboration with the client, family, physician and other service providers. Monthly telephone contacts and quarterly home visits are conducted to monitor service delivery and ensure client safety. MSSP coordinates and links individuals to counseling & support groups, health care resources, information & assistance, In-Home Supportive Services, medical equipment, minor home safety repairs, money management, nutrition services, respite and transportation.

Linkages Care Management Program is a federally funded program offered locally by the Fresno-Madera Agency on Aging. There is no income criteria and no mandatory fee for service. The program provides services to everyone regardless of income level. Linkages eligibility requires an individual to be age 60 or older, reside in Fresno or Madera county, able to be maintained in the community through the use of Care Management Services and have a need for care management and be willing to participate in the program. Linkages provides comprehensive care management that includes a thorough assessment of needs, a client driven plan of care, coordination of services, advocacy, ongoing support and education for clients and caregivers. Linkages assessment takes place in the client's home. Monthly telephone contacts and quarterly home visits are conducted to monitor service delivery and ensure client safety. This program links older adults to healthcare resources, adaptive equipment, nutrition programs, counseling services, minor home repairs, recreation activities, transportation, housing assistance and caregiver respite.
